The Effectiveness of Levosimendan on Veno-Arterial Extracorporeal Membrane Oxygenation Management and Outcome: A Systematic Review and Meta-Analysis

چکیده
ObjectivesVeno-arterial extracorporeal membrane oxygenation (VA-ECMO) provides a temporary support system for patients with cardiogenic shock refractory to conventional medical therapies. It has been reported that levosimendan may facilitate VA-ECMO weaning and improve survival. The primary objective of this review was examine the effect use on mortality in critically ill VA-ECMO.DesignMEDLINE, EMBASE, CENTRAL were searched. A pair reviewers identified eligible clinical trials. Two extracted data independently assessed risk bias. random-effect model used combine data. outcome success from VA-ECMO.Measurements Main ResultsSeven studies observational design, including total 630 patients, selected final analysis. sample size ranged ten-to-240 mean age between 53 65 years, more than half them underwent cardiac surgeries. durations varied four 11.6 days. Overall, significantly associated successful compared control (odds ratio [OR] 2.89, 95% CI, 1.53-5.46; poverall = 0.001); I2 49%). For survival, six (n 617) included meta-analysis involving 326 group 291 comparator group. Pooled results showed higher survival rate (OR 0.46, 0.30-0.71; 0.0004; 20%).ConclusionsLevosimendan therapy benefit or postcardiotomy needing severe cardiocirculatory compromise. To date, there is limited literature absence evidence randomized trials addressing weaning. This study be considered hypothesis-generating research controlled confirm its findings. Massive haemoptysis on veno-arterial extracorporeal membrane oxygenation.
A 49-year old female presented with severe heart failure with end-organ dysfunction and was placed on veno-arterial extracorporeal membrane oxygenation (ECMO) as a bridge to a decision for end-organ recovery. While on ECMO, the patient developed massive haemoptysis after a Swan-Ganz catheter manipulation. The haemoptysis was not controllable by conventional methods including bronchoscopy with c...
متن کاملDisrupting differential hypoxia in peripheral veno-arterial extracorporeal membrane oxygenation
Patients receiving circulatory support with peripheral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) are at risk of developing differential hypoxia. This phenomenon occurs in patients with concomitant respiratory failure. Poorly oxygenated blood, ejected into the ascending aorta from the left ventricle, competes with retrograde flow from the ECMO circuit, potentially causing myoca...
متن کاملWeaning Strategy from Veno-Arterial Extracorporeal Membrane Oxygenation (ECMO)
Background: Significant advances in extracorporeal technology have led to the more widespread use of veno-arterial extracorporeal membrane oxygenation (VA ECMO) for cardiac failure. However, procedures for weaning from VA ECMO are not standar‐ dized. High death rate after successful weaning shows that many questions remain unresolved in this field. Objectives: In this review, we discuss data fr...
متن کاملLeft ventricle unloading during veno-arterial extracorporeal membrane oxygenation
Extracorporeal membrane oxygenation in veno-arterial configuration (VA-ECMO) is a method widely used to support circulation during the most severe conditions of heart failure or refractory cardiac arrest. It frequently bridges this otherwise fatal status to recovery, therapeutic intervention or other circulatory support. However, the use of VA-ECMO in critical situations of severely impaired he...
متن کاملOutcome of Veno-Arterial Extracorporeal Membrane Oxygenation for Patients Undergoing Valvular Surgery
BACKGROUND We evaluated retrospectively the early and midterm results of using veno-arterial extracorporeal membrane oxygenation (VA-ECMO) support in patients undergoing valvular surgery. METHODS A total of 87 patients undergoing valvular surgery received VA-ECMO due to refractory postcardiotomy cardiogenic shock (PCS), who were eligible for inclusion were enrolled in this study. Preoperative...
